Disease/Disorder question for Scriptwriting purposes?

I'm in the process of developing a pilot for a TV series idea I had in mind (mainly for fun, but also for pitching if I can develop something I'm actually proud of).

I'm currently developing one of my characters (antagonist), and I want to give him a disease or disorder (never though I'd say that in a sentence).

My requirements so far is that it has to be:
– Non-infectious
– Eventually terminal (can survive for many years, even if only a few reported case have indicated so)
– even though it is 'killing him', from an onlookers point-of-view, the symptoms would not be obvious – subtle.

I have done a bit of research so far, and my best answer yet is 'Mesothelioma', which is a rare form of lung cancer, but I believe the symptoms may be a bit too obvious and debilitating. so any answers that are not 'research it yourself' would be very much appreciated!
